California Apostille Request Form

Apostille California ServiceNo matter where you are located in California, The United States, or any country in the world, you’ll need to mail-in an original or a certified copy of your documents before they are eligible for the California Apostille.  You can download our California Apostille request form by visiting this link: http://www.apostille.la/order-form/

The California Secretary of State will not authenticate copies of documents or documents which have been altered/damaged.  The following documents will either need to be an original or a certified copy signed by the County Clerk’s office: Birth Certificate, Death Certificate, Marriage Certificate, and Single Status Affidavit.  Documents issued by the County Clerk do not require notarization.  If you don’t have your certificate, you can order one by visiting VitalChek.com or contact any of the 58 County Clerks in California.

Certain documents require notarization prior to an Apostille.  These documents include: Power of Attorney, Affidavit, Travel Consent Letter, Diplomas & Transcripts (High School, College, University), Copy of a Passport and a Copy of a California Driver License.  It’s extremely important that the notary who notarizes your documents uses the correct California Acknowledgement, California Jurat, or California Copy Certification form.  We can email you these forms in order to avoid any mistakes the local notary may make.  Notaries who are members of the National Notary Association have access to these forms online.

We have a list of Documents which require an Apostille.  The Country requesting your forms will provide you a list of what is required.  For example, if you are getting married in Mexico, the Mexican Government will ask for your California Birth Certificate to be Apostilled.  If you plan to work in South Korea, the Korean Government will ask for an FBI Background Check Apostille.  It’s important to know what is being requested in order to avoid any delays and/or additional expenses.

Before mailing your documents to our office, please Contact Us.  Certain documents may not qualify for an Apostille.  It’s important that we review your documents over the phone or by email.  Thank you.
